An Asthma attack can be a very frightening and potentially fatal event. Every second counts when recognizing and treating an Asthma attack. In order for the staff at Open Door Christian Schools to provide safe, fast, effective medical care to your child in the event of an emergency we need to have the following form(s) completed and signed by both you and your child’s physician.
If your child carries and self-administers an inhaler then the following forms are required:
- Emergency Action Plan for Asthma
- Authorization for Student Possession and Use of an Asthma Inhaler
*We strongly encourage any child competent in the administration of their medication to carry and self-administer that medication as prescribed by their physician.
If you would like your child’s inhaler to be kept in the clinic and administered by a trained ODCS staff then the following forms are required:
- Emergency Action Plan for Asthma
- Medication Authorization Form
Please return the form(s) to the Nurse’s office or have them faxed to (440) 284-6033. All medical forms need to be returned and on file at the school prior to the first day of classes. If you have any questions or concerns please feel free to contact the clinic at (440) 326-2376. |
